How to get publishing/reprinting writes for somebody elses photos for a book ...

I’m attempting to write my first book. If I want to publish a photo in my book that I either don’t own or that was published somewhere else, how do I get rights/permission to print it? Is there a standard form I need to use? Do I have to pay the owner? How does this work? Thanks!
